Keeping in Touch Consent Form

This is the Keeping In Touch (Consent Form), required under GDPR.  It is used when people join a mailing list or share their contact details with us for example: to join a rota; a small group; or when leaving children with us for Messy Church etc.

ER Form

This is the Electoral Roll form. Each parish (Hook and HMR) have its own Electoral Roll, and you may join both.  If you live outside the parish, you will need to be a regular attendee.  If you are wanting to join our Electoral Roll you may also want to complete a Keeping in Touch form.  Please talk to the ER Officer or a member of the clergy for more information.

Keeping in Touch Consent Form This is the Keeping in Touch (Consent Form), required under GDPR to add people so that we can contact them, to be used when we add people to a mailing list Hook ER Form 2022 This is the 2022 Electoral Roll form for Hook - it includes a copy of the Keeping In Touch form (as we would expect most people on the Electoral Roll to be on our email list). HMR ER Form 2022 This is the 2022 Electoral Roll form for HMR - it includes a copy of the Keeping In Touch form (as we would expect most people on the Electoral Roll to be on our email list).
